Job Opening: Case Coordinator
BASIC FUNCTION
Responsible for coordinating all aspects of client interaction including explaining the appointment process, conducting assessments, creating individualized service plans with families, referrals for services, and providing follow up support and crisis assistance. Assist with intake calls for requests for services, present victim services information to new families, provide crisis counseling, and be an advocate to all clients and their families.
ESSENTIAL JOB FUNCTIONS
1. Responsible for initiating a relationship of trust with the child and family, administer the appropriate assessments and guiding the family through initial and/or follow up appointments.
2. Provide support, assistance, and intervention as needed with the child and/or family during and following appointments.
3. Inform the child and the family of the services available to them and complete the appointment with a wrap up session, utilizing appropriate consent and release of information forms.
4. Complete Client Assessments for all clients assigned to caseload, and return completed forms to administrative assistant for data entry. Administer reassessments to clients as necessary.
5. Conduct 45 day follow up phone calls to all families on caseload to assess any unmet needs, inquire about services received since initial appointment, and complete a brief agency satisfaction survey.
6. Assist in training and supervising volunteers, interns and/or service learning students when appropriate.
7. Facilitate presentations regarding the agency to community groups as requested.
8. Attend monthly Client Services Team meetings, CARE Team meetings, and individual supervision. Attend community meetings related to victim services.
9. Conduct intakes, both phone and walk-in.
10. Provide information to the families concerning agency services as well as the public (investigative) agencies they may encounter. Verify information provided by the caregiver.
11. Assist families to complete the application for financial support from the Victims of Crime Compensation fund.
12. Work with CPS to coordinate medical/forensic support to all CPS clients. Serve as liaison for families with the investigative agencies providing services to clients.
13. Provide statistical information as requested to track client services.
14. Assist client with emergency assistance, including housing, utilities or relocation services. Coordinate taxi service for medical appointments.
15. Follow-up Santa Rosa and Methodist Hospital referrals that have been identified for further medical services.
16. Provide crisis counseling for families as needed either by phone or during the appointment process.
17. Other duties as assigned by Client Services Director.
RELATIONSHIPS AND CONTACTS
Reports to: Client Services Director
Organizational Relationships: Works closely with all agency staff, including medical personnel, counselors, and CARE staff in providing services to clients.
JOB SPECIFICATIONS
1. Able to engage families and children and gain their trust to ensure participation in services.
2. Excellent organizational, writing and oral skills required.
3. Computer literate in all Microsoft Office products, specifically in the use of spreadsheets, databases and presentation software.
4. Bilingual preferred.
5. Able to act as a liaison for a variety of community organizations.
6. Bachelor’s degree and three years experience in social services.
7. Knowledge of child abuse and neglect dynamics, and experience working with children and families.

Office environment.
HOURS/SHIFTS
8:00am – 5:00pm Mon-Fri.
